A More Informed Starting Point for Assisted Living
CalAIM Assisted Living Facility Transitions is a Community Support offered by participating Medi-Cal managed care plans. Eligibility, service authorization, care-setting fit, and availability are reviewed individually.
Through CalAIM, participating Medi-Cal plans may authorize Assisted Living Facility Transitions for eligible members after reviewing care needs, program requirements, and the proposed setting. Room and board remains separate.

CalAIM is California’s statewide approach to helping Medi-Cal members access care in settings that may better fit their needs. Assisted Living Facility Transitions is one Community Support option offered through participating health plans.
The health plan determines eligibility and authorizes services. Room and board is separate from the covered service portion, and the Assisted Living Waiver is a different program with its own requirements.
