Our website forms and ordinary email help visitors begin a general conversation. They are not intended for medical records or other sensitive health information.
Do not include medical records, diagnoses, medication lists, health-plan member IDs, Social Security numbers, financial account information, or other sensitive information in a public form or ordinary email.
When documents are needed
Call or text 858-222-2787. A Zeal team member can explain the next step and provide an approved secure method when documents are necessary for a transition inquiry. Sending an initial general inquiry does not create a provider-patient relationship or guarantee eligibility, authorization, admission, or placement.
About HIPAA
Zeal works with healthcare organizations and may have privacy and security duties under applicable law or contract depending on the service and information involved. We do not describe the public website form or ordinary email as a HIPAA-secure document channel. This page is practical communication guidance and is not a health plan’s or healthcare provider’s formal Notice of Privacy Practices.
